A mechanical scarificator, in a rare form, retaining its original, leather-covered, case. Maker is “W&H HUTCHINSON SHEFFIELD”. The form of the mark would indicate that the instrument was likely made between 1849 and 1859. Good working order. Some might want to polish it, and it will polish up nicely, but I prefer to leave it as found.
